Legend DDD Services partners with Arizona’s Division of Developmental Disabilities (DDD) to empower individuals with developmental disabilities, such as autism, Down syndrome, cerebral palsy, epilepsy, and intellectual disabilities, to lead self‑directed, healthy, and meaningful lives. As a state‑contracted provider, we deliver a full range of supports across home and community settings, including in‑home habilitation, respite care, day‑programs, employment assistance, and residential services. Our compassionate team helps families and individuals navigate eligibility, coordinate care, and access the services they need to thrive
Services We Offer
Day Programs
At Legend DDD Services, our day programs provide a safe, engaging environment where individuals with developmental disabilities build life skills, socialize, and participate in community activities. We focus on promoting independence, confidence, and meaningful daily experiences.
Nurse Supported Group Homes
Our homes offer a safe, family-like environment where residents receive help with daily living tasks, medication management, and medical monitoring, all while promoting independence, community involvement, and quality of life. The added nursing support ensures that individuals with complex health conditions receive the specialized care they need in a comfortable, home-style setting.
Traditional Group Homes
At Legend DDD Services, our traditional group homes offer a supportive, family-style living environment for individuals with developmental disabilities. With 24/7 staff, we provide personalized care, help with daily living skills, and encourage independence, community involvement, and a sense of belonging in a safe, comfortable home setting.
